Living well with dementia through delaying or preventing disability and enhancing quality of life has become a primary objective of public health policy across the world. This paper discusses a co-design approach applied to a project which aims to develop a digital toolkit to support someone living with mild-to-moderate dementia, together with their informal carer(s), to live independently for longer in their own homes through planning and monitoring personalised care goals, engaging in daily and meaningful activities, and encouraging and facilitating shared decision making. The paper will focus on the implementation of design recommendations arising out of the requirements gathering stage as well the co-design of the application involving both person living with dementia/informal carer dyads as well as health professionals.
