SMILE
SMILE (Supporting Multi-morbidity self-care through Integration, Learning and eHealth) supports people living with multiple chronic conditions to manage their health and wellbeing at home. Originally funded through the Sláintecare Integration Fund, the project brought together Caredoc, NetwellCASALA at Dundalk Institute of Technology and the Trinity Centre for Practice and Healthcare Innovation at Trinity College Dublin. SMILE combines the ProACT digital health platform with nurse-led remote assessment and triage, supporting people with conditions such as diabetes, chronic obstructive pulmonary disease (COPD), heart failure and heart disease. Participants use connected devices to record health readings, while the ProACT app helps them review their information and access education to support everyday self-management.
NetwellCASALA’s contribution centres on the development and implementation of ProACT, bringing our expertise in accessible, person-centred digital health into community care. The platform enables Caredoc nurses to review participants’ readings, identify changes requiring attention and provide advice or arrange further clinical support. Building on the original project, SMILE 2 connects this approach with the HSE’s Enhanced Community Care programme and Chronic Disease Community Specialist Teams. Together, these partnerships aim to strengthen self-management, support earlier intervention and reduce avoidable hospital care, demonstrating how digital technology and professional support can help people receive care closer to home.
