ProACT provides home-based monitoring, personalised feedback, symptom-stabilisation tools, and structured care-network communication. ProACT is a modular, cloud-based platform, comprising an application for individuals and care network members (the ProACT App) and a secure administrative and triage interface for healthcare teams (Netwell Admin). The ProACT App provides personalised dashboards showing symptom trends, well-being measures and alerts. It integrates data from a range of connected devices and self-report tools, allowing users to monitor symptoms, goals, sleep, activity, weight and medication use. Notifications and contextual educational content support behaviour change and engagement. The App is intuitive and accessible for older adults, with versions for carers and healthcare professionals to support shared understanding of a user’s health. 

Netwell Admin gives care teams real-time access to symptom alerts, engagement patterns, and individual and cohort-level trends. Inspection tools present longitudinal overviews and engagement analytics, supporting operational decision-making. Features including multi-factor authentication, structured archiving, and exportable data reports.
